Forms for ENF Chairs

This page provides the forms you’ll use throughout the year to promote the ENF. Not sure what remit to submit? Follow this handy flowchart to help you choose the best form for your donations.

Forms to Submit Donations are official ENF remittances, to be used to submit donations to the ENF. Forms for Chair use only are forms that you will complete and submit to the ENF. Forms to share with members are forms that you can print and share with your members to collect donor information.

PLEASE NOTE: The final day of the 2024 Fiscal Year, March 31, falls on a Sunday. The ENF office is closed Friday, March 29, for Good Friday, and the ENF office is closed on the weekend. No ENF staff will be in the office to take credit card donations over the phone after 4:30 p.m. CT on March 28.

Forms to Submit Donations:

  • These online forms takes minutes and increases accuracy. Use them in place of traditional remittance sheets, spreadsheets or Donor I.D. Reports.
    • Online Remittance Form: Use this online remittance form to submit donor information to the ENF, followed by mailing a check to our office. Click here for instructions. NOTE: Donations are credited the day the check arrives in the ENF office, not the day the online remittance is submitted. Please mail a printed version of the report and the Batch ID with the check to speed up processing time.
    • CLMS Remittance Form for ENF Donations made with Dues: Work with your Lodge Secretary to use this CLMS dues donations remittance report to submit donations that were collected with dues, followed by mailing a check to our office. For help navigating this process check out this helpful visual. Only Lodge Secretaries, through CLMS, can run the report. Instructions can be viewed here. NOTE: Donations are credited the day the check arrives in the ENF office, not the day the online remittance is submitted.
  • Remittance Form Templates: Use these printable remittance forms to organize and submit donations to the ENF. This template also includes sample forms and instructions on how to properly complete them.
  • ENF Donor Card: Donor cards allow you to capture all necessary information from your donors, which in turn, helps the ENF provide better service; always have them on hand, especially for reporting new donors.

Forms for Chairs:

  • Supply Order Form: ENF provides supplies for you and your Lodge free of charge. Use this form to submit your order online now.

Forms to Share with Members:

  • ENF Donor Card: Donor cards allow you to capture all necessary information from your donors, which in turn, helps the ENF provide better service; always have them on hand, especially for reporting new donors.
  • Fidelity Club Enrollment Form: Save time and stamps by encouraging donors to use this form to enroll in the ENF Fidelity Club, the recognition club for recurring givers.
  • John F. Malley Society Application: Provide this application to donors who have planned a gift to the ENF so they may enroll in the John F. Malley Society.
  • Named Scholarship Enrollment Form: Use this form to enroll donors in the MVS Named Scholarship Program to connect personally with a scholarship recipient.
  • Pulse Sign-Up Sheet: Use this sheet to collect email addresses from members of your Lodge for Pulse, our monthly email newsletter.
  • Tell Us More Form: ENF wants to get to know volunteers, donors and friends of the ENF as best we can. Use this printable form to collect information from Lodge members.

Submitting a Lodge Donation with a Credit Card:

  • Using a Lodge or personal credit card is a fast and easy way to submit ENF donations without having to go through the process of acquiring a check.
