Leave with bartender only if the object is too large for the slot. APPLICATIONS - Please assure the forms are completed in their entirety in a legible manner. Sponsors are to complete the sections on the front and back. The $50.00 application fee must accompany the application. Unacceptable applications will be rejected, thus delaying the process.
