This form is used to request the use of the Lodge Facility for a special event. Requests MUST be submitted a minimum of 30 days in advance of the event. The requestor must be an Elks member in good standing and is responsible for the event, including Clean Up, Breakdown, and any applicable fees. Fees may be applied and will be determined by the House Committee. NOTE: Submission of this request DOES NOT constitute approval of event space. Requests will be reviewed by the House Committee and Exalted Ruler. The House Committee and Exalted Ruler must approve all events.
