How can I obtain an application for the Most Valuable Student Contest?

Applications are available through your local Elks Lodge, online at www.elks.org/enf/scholars/mvs.cfm, or by sending a self-addressed, stamped envelope to the Foundation at 2750 N. Lakeview Avenue, Chicago, IL 60614-2256. We cannot satisfy requests for applications that do not include self-addressed, stamped envelopes.
